What is Hypertension?

Hypertension, commonly known as high blood pressure, is a chronic medical condition in which the force of blood against the walls of the arteries remains consistently higher than normal. It develops when the heart pumps blood with excessive force or when the blood vessels become narrowed or less flexible, increasing resistance to blood flow. Often referred to as a “silent” condition because it may not cause noticeable symptoms, hypertension can progressively damage vital organs and increase the risk of serious complications, including heart disease, stroke, kidney disease, and other cardiovascular disorders. Regular blood pressure monitoring and appropriate lifestyle modifications and medical management are important for controlling blood pressure and reducing the risk of long-term complications.

Stuti Mahajan, consulting manager at DelveInsight, said that the hypertension treatment landscape is evolving beyond conventional antihypertensive therapies, with increasing development of novel mechanisms and longer-acting treatment approaches focused on uncontrolled and resistant hypertension, treatment adherence, and cardiovascular-risk reduction. The pipeline reflects growing diversification, including RNA-interference therapies targeting angiotensinogen, activin-signaling inhibition, emerging oral therapies, and pulmonary vascular-targeted approaches, while lorundrostat represents a leading opportunity among novel conventional antihypertensive agents. The advancement of zilebesiran and other mechanism-specific therapies highlights the potential of durable and infrequently administered treatments to improve adherence and sustained blood-pressure control, although clinical setbacks with emerging agents demonstrate the challenges of translating novel mechanisms into meaningful outcomes. Overall, the hypertension market outlook remains positive, with future commercial success likely to depend on durable efficacy, cardiovascular and renal outcomes, safety, adherence benefits, and clear differentiation from established low-cost generic therapies.

Recent Developments in Hypertension Treatment Space

  • In August 2026, Gossamer Bio, Inc. , a clinical-stage biopharmaceutical company focused on the development and commercialization of seralutinib for the treatment of pulmonary arterial hypertension (PAH) and pulmonary hypertension associated with interstitial lung disease (PH-ILD), announced that it has entered into a securities purchase agreement with certain new and existing institutional investors for a private placement financing expected to provide aggregate gross proceeds of up to approximately USD 250 million, before deducting placement agent fees and estimated offering expenses. The private placement consists of an initial closing, a committed second closing contingent upon FDA acceptance of the seralutinib new drug application (NDA) in PAH (the NDA Acceptance Milestone) and warrants exercisable upon FDA approval of seralutinib in PAH (the FDA Approval Milestone). The financing follows the Company’s recent reacquisition of worldwide rights to seralutinib and is in advance of the planned NDA submission in September 2026.
  • In June 2026, CRISPR Therapeutics mentioned in its second quarter financial results that CTX340™, targeting angiotensinogen (AGT), received IND clearance from the FDA. The Company has initiated a Phase I clinical trial for patients with refractory hypertension.
  • In May 2026, CalciMedica in its second quarter financial results provided an update, that studies are ongoing to further characterize the pharmacology, pharmacokinetics, and safety profile of CM5480, a proprietary oral CRAC channel inhibitor candidate with the potential to be a first-in-class, differentiated therapy targeting pulmonary vascular and right ventricular remodeling, key drivers of disease progression in PH, including pulmonary arterial hypertension. An IND submission for CM5480 is anticipated in 2027.
  • In May 2026, Mineralys Therapeutics, Inc. presented new clinical data for lorundrostat at the 35th European Meeting on Hypertension and Cardiovascular Protection (ESH 2026) in Gdańsk, Poland. The analysis evaluated the efficacy and safety of once-daily lorundrostat 50 mg by CKD status among 800 participants with uncontrolled or resistant hypertension enrolled in the randomized, double-blind, placebo-controlled Phase III Launch-HTN trial. Among participants with CKD (n=192), 71% were receiving three or more anti-hypertensive medications at baseline, compared with 56% of those without CKD. In addition, 31% of participants with CKD had systolic blood pressure (SBP) ≥160 mmHg at baseline, versus 17% of participants without CKD. Lorundrostat demonstrated significant reductions in SBP that were comparable between CKD and non-CKD participants. At week 12, placebo-adjusted SBP reductions were 9.6 mmHg in participants with CKD (p=0.0022) and 12.2 mmHg in those without CKD (p<0.0001). A greater proportion of lorundrostat-treated participants also achieved target SBP of <130 mmHg at week 12 compared with placebo, both among CKD participants (44% vs 18%) and non-CKD participants (48% vs 22%).
  • In April 2026, AllRock Bio, Inc. announced the dosing of the first patients in the Phase IIa ROCSTAR trial of ROC-101, a first-in-class, oral pan-rho-associated protein kinase (ROCK) inhibitor, in pulmonary hypertension (PH). Building on Phase I data, the open-label, multicenter, exploratory study will evaluate the safety, tolerability, and efficacy of ROC-101 to treat patients with pulmonary arterial hypertension (PAH) or PH associated with interstitial lung disease (ILD-PH).
  • In March 2026, Kardigan, a clinical-stage precision therapeutics company developing medicines that target the root cause of specific cardiovascular diseases where no approved treatments exist, announced results from KARDINAL, a Phase II clinical trial evaluating the effects of tonlamarsen in patients with uncontrolled hypertension despite treatment with two or more antihypertensive medications. Data were presented at the American College of Cardiology’s Annual Scientific Session & Expo (ACC.26) and simultaneously published in The Journal of the American College of Cardiology (JACC). In the clinical trial, tonlamarsen, an investigational antisense oligonucleotide (ASO) targeting angiotensinogen (AGT), demonstrated a statistically significant reduction in plasma AGT levels and a clinically meaningful reduction in in-office systolic blood pressure (oSBP) from baseline to Week 20. These results highlight the intended biological effect of tonlamarsen and support evaluation in Kardigan’s planned Phase IIb clinical trial in patients with acute severe hypertension (ASH) post-hospitalization.
  • In March 2026, Mineralys Therapeutics, Inc. announced that the FDA has accepted the New Drug Application (NDA) for lorundrostat for the treatment of adult patients with hypertension in combination with other antihypertensive drugs. The FDA has assigned a Prescription Drug User Fee Act (PDUFA) target action date of December 22, 2026.
  • In February 2026, GSK plc announced that it has entered an agreement to acquire 35Pharma Inc., a Canada-based, private, clinical-stage biopharmaceutical company specialised in the development of novel protein-based therapeutics. The acquisition includes HS235, a potential best-in-class investigational medicine that has completed phase I healthy volunteer clinical trials with studies to start imminently in pulmonary arterial hypertension (PAH) and pulmonary hypertension due to heart failure with preserved ejection fraction (PH-HFpEF).
  • In October 2025, Alnylam Pharmaceuticals, Inc. announced that the first patient has been dosed in ZENITH (ZilebEsiraN CardIovascular OuTcome Study in Hypertension), a global Phase III cardiovascular outcomes trial (CVOT) designed to evaluate the potential of zilebesiran, an investigational biannual subcutaneously administered RNAi therapeutic, to reduce the risk of major adverse cardiovascular (CV) events in patients with uncontrolled hypertension. Zilebesiran works by targeting liver-expressed angiotensinogen (AGT), the most upstream precursor in the Renin-Angiotensin-Aldosterone System (RAAS), which plays a key role in blood pressure regulation and impacts CV and renal health.
  • In August 2025, Roche and Alnylam announced the decision to initiate a Phase III cardiovascular outcomes trial (CVOT) to evaluate the ability of zilebesiran, a RNAi therapeutic, to reduce the risk of major adverse cardiovascular events in patients with uncontrolled hypertension. This decision was informed by the comprehensive KARDIA Phase II programme, including KARDIA 1, KARDIA 2 and the most recent KARDIA-3 study evaluating the efficacy and safety of zilebesiran in patients with uncontrolled hypertension and high cardiovascular (CV) risk, on two to four standard of care antihypertensives. In particular, KARDIA-3 aimed to define the patient population to be investigated in the Phase III CV outcomes trial.
  • In March 2025, Mineralys Therapeutics, Inc. announced positive topline data from its pivotal Launch-HTN Phase III and pivotal Advance-HTN Phase II trials evaluating the efficacy and safety of lorundrostat for the treatment of uncontrolled hypertension (uHTN) or resistant hypertension (rHTN).
